Charting is the primary way by which healthcare professionals formally communicate their evaluation and management of patients after they present for medical attention. Unlike other clinical skills, there can be little explicit training of medical students around the task of charting, and a general perception that individuals learn charting on the job during their internship or residency training. However, charting is attracting greater attention given the relationship between individual and team factors and the outcome of diagnostic error. This presentation shares authentic case examples of charting, and suggests ways in which educators can use interventions for improving documentation and better communicate their diagnostic reasoning.

About Dr. Patel: Dr. Rakesh Patel is Clinical Associate Professor in Medical Education at the University of Nottingham, and Honorary Consultant Nephrologist at Nottingham University Hospitals NHS Trust. After completing a Masters in Medical Education at the University of Dundee in November 2011 and a professional doctorate (MD) in medical education at the University of Leicester, he was a NIHR Academic Clinical Lecturer in Medical Education (2012-2016) and explored the effectiveness of an evidence-based theory-driven approach for improving the performance of healthcare professionals in practice. He is currently Director of the PGCert/PGDip/MMedSci in Medical Education at the University of Nottingham and an Education Associate for the General Medical Council.
